緊急避妊の知識

コンドームは、避妊のほか性感染症の予防にも欠かせないが、破れたり、外れたりして避妊に失敗することがある。このほか、避妊しなかった、レイプ被害にあった、などの場合に行う最後の手段が緊急避妊だ。

　卵巣からの排卵、受精卵の着床といった妊娠の過程は、脳の視床下部や下垂体から分泌されるホルモンや、卵巣から分泌される女性ホルモンが相互に作用して成り立っている。緊急避妊は、性行為から一定期間内に、女性ホルモン剤（ピル）を服用し、この相互作用を崩して妊娠を阻む方法だ。

　なぜ避妊できるのか。詳しい仕組みは分かっていないが、〈１〉排卵の抑制や遅延を引き起こす〈２〉子宮内膜を着床しづらい状態にする〈３〉子宮頸管（けいかん）の粘液の分泌が変わり、子宮内に精子が進入しにくい環境にする――などの作用が考えられている。

　現在、国内で使われている方法は、女性ホルモンの卵胞ホルモンと黄体ホルモンを合成した中用量ピルを、性行為の後７２時間以内に１回、その１２時間後にもう１回、２錠ずつ服用する方法だ。考案した産婦人科医の名前から「ヤツペ法」と呼ばれ、１９７０年代から広まった。

　海外の研究では、ヤツペ法による妊娠率は３・２％。これは一般に、薬を使わない場合の４分の１程度に抑えられる計算になる。ただ、通常の避妊薬である低用量ピルより女性ホルモンの含有量が多く、吐き気や嘔吐（おうと）、倦怠（けんたい）感などの副作用がある。

　この中用量ピルは、強い生理痛などを伴う月経困難症や月経周期異常の治療薬として認められているものの、緊急避妊薬としては承認されておらず、費用は自費になる。施設によって差はあるが、平均５０００円ほどだ。

　一方、欧米では９０年代後半から、黄体ホルモン単独のピルを使った方法も登場した。ヤツペ法に比べ、黄体ホルモン単独剤は副作用が少なく、服用も１回で済み、妊娠率が低いとのデータがある。

　世界保健機関も推奨している方法なのに、まだ国内では承認されていない。メーカーが昨秋、承認を目指し、黄体ホルモン単独剤の臨床試験を始めた段階だ。

　国内では、年間約３０万件の人工妊娠中絶が行われているが、緊急避妊の認知度はまだ低い。厚生労働省研究班が先月まとめた男女３０００人へのアンケート調査では、緊急避妊を知っていたのは２４％にとどまった。

　日本家族計画協会クリニック（東京・市ヶ谷）所長の北村邦夫さんは「避妊するなら、日常的にコンドームや低用量ピルを使うのが大前提」とした上で、「緊急避妊の知識がないばかりに中絶せざるを得なくなり、心身とも傷つく女性も相当いるのではないか」と指摘している。
